原发性高血压病患者脑功能指标与睡眠质量关系研究

Relationship between Brain Function Indices and Sleep Quality in Patients with Primary Hypertension

摘要: 背景 睡眠障碍与高血压的发病风险、昼夜血压节律变化及并发症发病风险明显相关,因此对高血压患者的睡眠质量进行评估显得尤为重要。目的 探讨原发性高血压病患者脑功能指标与睡眠质量的相关性。方法 随机选取2017年7月—2019年1月在哈尔滨医科大学附属第一医院心血管内科门诊及住院患者中被诊断为原发性高血压病患者80例为研究对象,根据匹兹堡睡眠质量指数量表(PSQI)评分分为两组:睡眠质量差组(PSQI评分≥11分)40例;睡眠质量好组(PSQI评分≤10分)40例。比较两组脑功能指标水平(脑耗能、困倦+脑疲劳),并采用Pearson相关分析脑功能指标水平与PSQI评分之间的相关性。结果 睡眠质量差组脑耗能为(342.38±147.14),睡眠质量好组脑耗能为(211.03±116.07),两组比较差异有统计学意义(t=4.433,P<0.001)。睡眠质量差组困倦+脑疲劳为(58.78±29.23),睡眠质量好组困倦+脑疲劳为(43.73±19.96),两组比较差异有统计学意义(t'=2.329,P=0.027)。Pearson相关分析结果显示,脑耗能与PSQI总分呈正相关(r=0.409,P<0.001),困倦+脑疲劳与PSQI总分呈正相关(r=0.341,P=0.002)。结论 原发性高血压病患者的脑功能指标(脑耗能、困倦+脑疲劳)与睡眠质量相关,可在一定程度上反映患者睡眠质量。脑功能评估为原发性高血压病患者睡眠质量评估提供了一种更为便捷客观的测量方法。

Abstract: Background Sleep disorder is significantly associated with the risk of hypertension and hypertension-related complications,and circadian changes in blood pressure.Therefore,it is particularly important to evaluate the sleep quality in hypertensive patients.Objective To explore the relationship between brain function indices and sleep quality in patients with primary hypertension.Methods Eighty cases of primary hypertension were randomly selected from the cardiovascular outpatients and inpatients treated in the First Affiliated Hospital of Harbin Medical University during July 2017 to January 2019,and were divided into poor sleep quality group(n=40,PSQI score≥11) and good sleep quality group(n=40,PSQI score≤10).Brain function indices(brain energy dissipation,drowsiness value + brain fag) of the two groups were compared.The correlation between brain function indices and PSQI score was analyzed by Pearson correlation.Results The brain energy dissipation was(342.38±147.14) in poor sleep quality group and(211.03±116.07) in good sleep quality group,showing significant intergroup difference(t=4.433,P<0.001).The drowsiness value + brain fag was(58.78±29.23) in poor sleep quality group and(43.73±19.96) in good sleep quality group,demonstrating significant intergroup difference(t'=2.329,P=0.027).Pearson correlation coefficient analysis showed that brain energy consumption was positively correlated with the total score of PSQI(r=0.409,P<0.001),and the drowsiness value + brain fag was positively correlated with the total score of PSQI(r=0.341,P=0.002).Conclusion Brain function indices(brain energy dissipation,drowsiness value + brain fag) are related to sleep quality and can reflect the sleep quality in patients with primary hypertension.The assessment of brain function provides a more convenient and accurate method for the assessment of sleep quality in such patients.
