居家运动康复依从性水平对心房颤动患者射频消融术后复发情况的影响研究

摘要: 背景 虽然运动康复已被证明对心血管患者是安全有效的,但依从性并不乐观。运动是否产生持续的益处与依从性密切相关,但目前的研究未能描述运动依从性与射频消融患者房颤复发之间的关系。目的 探究心房颤动(房颤)患者射频消融术后复发情况的影响因素。方法 选取2020年5月—11月在南京市某三甲医院心血管内科行射频消融手术的246例房颤患者为研究对象,对全程参与五期调查的151例患者的相关数据进行统计分析,收集运动康复依从性(包括强度-时间依从性和频率依从性)、患者积极度、心脏康复需求及一般资料等情况。采用广义估计方程探讨房颤患者射频消融术后复发情况的影响因素。结果 本研究共有23例患者复发。广义估计方程结果显示,BMI、吸烟、房颤类型、积极度、过程焦虑、结果焦虑和强度-时间依从性是复发情况的影响因素。结论 房颤患者射频消融术后居家运动康复依从性逐期改变,建议临床医务人员重视运动强度的指导和监督,采取针对性措施降低过程焦虑和结果焦虑,以期更加有效地通过运动康复改善患者预后。

Abstract: Background Although exercise rehabilitation has been proven to be safe and effective for cardiovascular patients, adherence is not optimistic. Whether exercise produce sustained benefits was closely associated with adherence, but current studies failed to depict the relationship between exercise adherence and recurrence of atrial fibrillation in patients undergoing radiofrequency ablation procedure.Aim Exploring the influencing factors on recurrence of atrial fibrillation (AF) in patients undergoing radiofrequency ablation procedure. Methods A total of 246 atrial fibrillation patients who underwent radiofrequency ablation surgery in a tertiary hospital in Nanjing from May to November 2020 were selected as the study subjects. Statistical analysis was conducted on the relevant data of 151 patients who participated in all five phases of the investigation, including adherence to exercise rehabilitation (including intensity-time adherence and frequency adherence), Patient activation measure13, Cardiac Rehabilitation Inventory, and general information. The generalized estimating equation was used to explore the influencing factors on post-ablation recurrence in atrial fibrillation patients.Results This study had a total of 23 patients who experienced recurrence. The results from the generalized estimating equation showed that BMI, smoking, type of atrial fibrillation, activation level, process anxiety, outcome anxiety and intensity-time adherence were all factors influencing the recurrence situation. Conclusions The adherence to home-based exercise rehabilitation in patients with atrial fibrillation after radiofrequency ablation changes progressively. It is recommended that healthcare professionals pay attention to guiding and supervising exercise intensity, take targeted measures to reduce process anxiety and outcome anxiety, in order to effectively improve patient prognosis through exercise rehabilitation.
