单核细胞 / 高密度脂蛋白胆固醇与高尿酸血症患病率的
关联及性别差异研究

【摘要】 背景 高尿酸血症(HUA)是一种常见的代谢性疾病,可对全身多个靶器官造成损害。研究证实
HUA 与炎性标志物密切相关,单核细胞 / 高密度脂蛋白胆固醇(MHR)作为一种新型炎性标志物在社区易获得,研
究 MHR 与 HUA 间的关联对社区 HUA 防治工作有重要意义。目的 研究 MHR 与 HUA 的关联,探讨 MHR 作为一种
新型炎性标志物的价值及性别差异。方法 收集参加 2020 年上海市浦东新区老港镇 50 岁以上中老年人体检的人群的
一般资料及检验数据,计算 MHR。共纳入体检信息完整的中老年人 12 305 例,其中男 5 498 例(44.7%),女 6 807
例(55.3%)。HUA 定义为女性血尿酸(SUA)>360 μmol/L,男性 SUA>420 μmol/L。采用多因素 Logistic 回归分析
探究 HUA 与 MHR 的关系,并按性别分层。结果 本研究中老年人 HUA 总患病率为 15.8%(1 948/12 305),男性为
17.7%(974/5 498),女性为 14.3%(974/6 807)。多因素 Logistic 回归分析结果显示,单核细胞(MONO)〔男性:
OR=2.010,95%CI(1.265,3.194);女性:OR=5.427,95%CI(3.059,9.627)〕、MHR〔男性:OR=1.918,95%CI(1.111,
3.313);女性:OR=6.073,95%CI(2.984,12.358)〕是男性和女性发生 HUA 的独立影响因素(P<0.05)。结论
男性和女性 MHR 与 HUA 的发生风险均呈独立相关,与男性相比,MHR 与 HUA 患病率的相关性在女性中更显著。

【Abstract】 Background Hyperuricemia (HUA) is a common metabolic disease,which may cause damage to
multiple target organs. Studies have showed that HUA is strongly related to inflammatory markers. It is of great importance to study
the relationship of HUA with monocyte to high-density lipoprotein cholesterol ratio(MHR),a new inflammatory marker that
could be obtained easily in the community,for community-based prevention and treatment of HUA. Objective To examine
the overall and sex-specific associations between MHR and HUA,to assess the predictive value of HUA for HUA. Methods
The demographic and 2020 physical examination data of 12 305 subjects over 50 years old〔5 498 men(44.7%) and 6 807
women(55.3%)〕 from Laogang community of Shanghai's Pudong New Area were analyzed retrospectively. HUA was defined as
serum uric acid level > 360 μmol/L in women,and >420 μmol/L in men. Multivariate Logistic regression was used to examine
the association between MHR and HUA in all participants,and in men and women,respectively. Results The prevalence of
HUA in all participants,men and women was 15.8%(1 948/12 305),17.7%(974/5 498),and 14.3%(974/6 807),
respectively. After adjusting for confounding factors,monocyte〔male,OR=2.010,95%CI(1.265,3.194);female,
OR=5.427,95%CI(3.059,9.627) 〕 and MHR〔male,OR=1.918,95%CI(1.111,3.313);female,OR=6.073,
