摘要: 背景 随着慢性非传染性疾病(以下简称“慢性病”)防控重心向社区前移,基层医疗卫生机构在营养健康促进及慢性病早期干预中的作用日益凸显。为破解当前基层营养门诊建设及多学科协作服务能力普遍薄弱的难题,亟需探索切实可行的医防融合新路径。目的 总结N-RCSP精准营养健康管理模式(营养、康复、中医、运动、心理)的实践经验,为基层医疗卫生机构规范开展基于医防融合的营养门诊建设与管理提供科学、可操作的指导框架。内容 本共识基于文献回顾与专家讨论,系统构建了以N-RCSP“五位一体”干预策略为核心的精准营养健康管理体系。文件详细明确了基层营养门诊的建设原则、人员及设备配置、信息化标准,并梳理了涵盖“筛查-评估-干预-监测-随访”的数字化闭环服务流程。结论 N-RCSP精准营养健康管理模式是落实医防融合的有效抓手,能全面提升基层医疗机构的慢性病非药物干预与健康管理水平。推广该模式有助于降低基层全科医生实施专业营养干预的技术门槛,促进多学科协作。

Abstract: Background With the center of gravity for the prevention and control of chronic non-communicable diseases (hereinafter referred to as "chronic diseases") moving closer to the community level, primary healthcare institutions are playing an increasingly prominent role in nutrition health promotion and early intervention of chronic diseases. To solve the current weak capacity for building nutrition clinics and providing multidisciplinary services at the primary level, there is an urgent need to explore practical new paths for integration of clinical care and public health.Objective To summarize the practical experience of the N-RCSP precision nutrition health management model (Nutrition, Rehabilitation, Chinese medicine, Sports, Psychology) and provide a scientific, operable guiding framework for primary healthcare institutions to standardize the construction and management of nutrition clinics based on the integration of clinical care and public health.Content Based on literature reviews and expert discussions, this consensus systematically constructs a precision nutrition health management system centered on the N-RCSP “five-in-one” integrated intervention strategy. The document specifies the construction principles, personnel and equipment allocation, and informatization standards for primary nutrition clinics, and outlines a digital closed-loop service process covering “screening-assessment-intervention-monitoring-follow-up”.Conclusion The N-RCSP precision nutrition health management model helps lower the technical threshold for primary general practitioners to implement professional nutritional interventions and promotes multidisciplinary collaboration. As an effective tool for implementing the integration of clinical care and public health, it comprehensively enhances the non-pharmacological intervention capacity for chronic diseases and health management levels in primary healthcare institutions.
