Interpretation of 2021 ACG Clinical Guideline: Upper Gastrointestinal and Ulcer Bleeding

摘要: 上消化道出血(UGIB)是临床常见的急重症之一。近年来,随着国内外相关指南不断更新,UGIB患者的管理逐渐规范,但在临床处理过程中仍有难点尚待解决。2021年5月美国胃肠病学会在2012年《溃疡出血患者管理指南》的基础上进行了更新,针对溃疡引起的UGIB患者的初始管理、内镜评估时间、内镜治疗方式和内镜治疗后管理等方面提供了新的临床指导建议。本文对2021年指南的主要内容及重点更新内容进行解读,并结合国内外最新指南进行对比,以期为临床治疗UGIB提供诊疗参考。

Abstract: Upper gastrointestinal bleeding(UGIB) is one of the common acute gastroenterological emergencies. The recent updates of guidelines for the management of UGIB have gradually promoted the standardization of UGIB treatment. However,there are still many difficulties to be solved in clinical management. In May 2021,the American College of Gastroenterology (ACG) updated the 2012 guideline for the management of patients with ulcer bleeding,providing new clinical recommendations for the initial management,timing of endoscopic evaluation,endoscopic treatment,and post-endoscopic management of patients with UGIB. We interpreted the essentials of the 2021 guideline,with a detailed analysis of the key updates,and compared the guideline with other latest guidelines in this field,aiming to provide a reference for clinical diagnosis and treatment of UGIB
