克罗恩病伴右下腹痛、面部肿胀、肺部病变的肠外表现一例报道并文献复习

摘要: 克罗恩病是一种病因不明的慢性肉芽肿性疾病,主要累及小肠及回盲部,部分患者伴肠外表现。克罗恩病的肠外表现可累及各系统,但没有关于面部肿胀的报道。本文报道了1例右下腹痛、面部肿胀、肺部病变的克罗恩病伴肠外表现患者,并进行文献复习,提出其确诊注意鉴别梅-罗综合征和口面部肉芽肿病,二者均与克罗恩病的临床和病理学相关;其次需注意鉴别药物(美沙拉嗪或柳氮磺胺吡啶)引起的肺部病变;旨在增加临床医生对克罗恩病伴肠外表现的进一步认识。

Abstract: Crohn's disease is a chronic granulomatous disease with unknown etiology involving the small intestine and ileocecal region.Some patients had parenteral manifestations.The extraintestinal manifestations of Crohn's disease may involve various systems,but there are no reports of facial swelling.A case of Crohn's disease with parenteral manifestations,including right lower abdominal pain,facial swelling and pulmonary lesions,was reported in this paper.Literature review was also carried out.We suggested that attention should be paid to the differential diagnosis between Melkersson's Rosenthal syndrome and oral-facial granulomatosis,both of which were related to the clinic and pathology of Crohn's disease.Secondly,attention should be paid to pulmonary lesions caused by drugs (mesalazine or sulfasalazine) so as to increase clinicians' understanding of Crohn's disease complicated with parenteral manifestations.
