Abstract:

Background

Exploring a cancer management plan suitable for our national conditions by utilizing the convenience of community services is an urgent need to ensure the out-of-hospital management for patients with cancer.

Objective

To analyze the current status, needs, and challenges in community-based cancer patient management, providing information for the development of innovative models for cancer patient management in Chinese communities.

Methods

A total of 35 cancer patients and 23 community healthcare professionals were recruited for in-depth interviews on the current status and needs of community cancer patient management from September to November 2023. Thematic framework analysis was used for data analysis. While 14 key decision-makers were selected for focus group interviews on the implementation factors of community-led cancer patient management models in December 2023, and data analysis was carried out based on grounded theory research methods.

Results

Cancer patients in the community rehabilitation stage not only faced various physical discomforts caused by the disease and treatment, but also suffer from psychological distress and social isolation. However, they held a negative attitude towards the existing community management. Primary healthcare institutions had already implemented various cancer management practices in their daily work, but the content is fragmented and lacked integration. A community-led, personalized, and proactive cancer patient management model was expected by stakeholders, and the "burdens" and "benefits" brought by its implementation were concerns of key decision-makers.

Conclusion

Currently, the management of cancer patients in the community has not yet formed a complete system, and the health needs of cancer patients in the community have not been fully met. Community-led personalized and proactive cancer patient management can be the direction for further exploration.

背景

利用社区便捷优势,探索适合我国国情的基层肿瘤患者管理方案,是保障肿瘤患者院外管理的迫切需求。

目的

从利益相关者视角多维度分析社区肿瘤患者管理现状、需求与挑战。

方法

2023年9—11月针对社区肿瘤患者管理现状与需求,对35例肿瘤患者和23名社区肿瘤管理医护人员进行深入访谈;2023年12月针对社区肿瘤患者管理模式核心要素,对14名肿瘤患者管理决策关键人士进行焦点小组访谈。分别采用主题框架法和扎根理论研究方法进行数据分析。

结果

肿瘤患者在社区康复阶段不仅面临疾病与治疗带来的各种生理不适,还伴随着心理上的困扰和社交上的隔离,且对现有基层卫生服务机构管理服务态度消极。基层卫生服务机构在日常工作中已开展多项肿瘤管理工作,但内容零散、缺乏有效整合。社区主导的个性化、主动化肿瘤患者管理被利益相关方所期待,模式所带来的"负担"和"效益"被肿瘤患者管理决策关键人士所关注。

结论

当前社区肿瘤患者管理工作尚未形成完整体系,社区肿瘤患者健康需求未得到满足,社区主导的个性化、主动化肿瘤患者管理可为下一步探索方向。
