Abstract:

Background

Cardiovascular disease (CVD) is one of the leading causes of death worldwide, and its incidence continues to rise with the increasing prevalence of metabolic disorders. The phlegm-dampness constitution is considered a potential risk factor for CVD; however, its association with CVD has not been sufficiently validated in epidemiological studies.

Objective

This study aimed to construct a novel variable, TANS01, to characterize the phlegm-dampness constitution and to investigate its association with the risk of CVD, thereby evaluating the cardiovascular risk among individuals with phlegm-dampness constitution.

Methods

We included 15 279 participants from seven survey cycles (2003-2016) of the National Health and Nutrition Examination Survey (NHANES) database. A binary indicator, TANS01, was constructed to represent the phlegm-dampness constitution. Epidemiological analysis was conducted to investigate the potential relationship between the novel TANS01 indicator and CVD incidence. Univariate and multivariate logistic regression analyses were employed to explore the association between TANS01 and various cardiovascular outcome variables.

Results

Based on the TANS01 classification, participants were divided into a phlegm-dampness constitution group (n=6 437) and a non-phlegm-dampness constitution group (n=8 842). Statistically significant differences (P<0.05) were observed between the two groups regarding age, age groups, sex, smoking, alcohol consumption, hypertension, overweight, dyslipidemia, dietary preferences, lack of exercise, heart failure, BMI, white blood cell count (WBC), red blood cell count (RBC), hemoglobin (Hb), glycated hemoglobin (HbA1c), fasting plasma glucose (FPG), low-density lipoprotein cholesterol (LDL-C), triglycerides (TG), aspartate aminotransferase (AST), alanine aminotransferase (ALT), uric acid (UA), and creatinine (Cr). Univariate Logistic regression analysis indicated that TANS01 was a risk factor for heart failure (OR=2.67, 95%CI=2.04-3.49, P<0.001), coronary heart disease (OR=2.04, 95%CI=1.66-2.51, P<0.001), and angina pectoris (OR=2.29, 95%CI=1.72-3.03, P<0.001). The association remained robust after adjusting for confounding factors (age, sex, smoking, and alcohol consumption) in the multivariate Logistic regression analysis. Subgroup analysis revealed that TANS01 was a risk factor for heart failure across different subgroups defined by sex, age, smoking, alcohol consumption, diabetes, and hypertension status (P<0.05), with no statistically significant difference in predictive efficacy among subgroups (Pinteraction>0.05). Similarly, TANS01 was a risk factor for coronary heart disease and angina pectoris across subgroups of sex, age, smoking, and alcohol consumption (P<0.05); however, the predictive value of TANS01 for coronary heart disease and angina pectoris was not statistically significant in patients with diabetes and hypertension (P>0.05).

Conclusion

The findings of this study demonstrate a significant association between TANS01 and the risk of CVD, including heart failure, coronary heart disease, and angina pectoris. This provides epidemiological evidence supporting the phlegm-dampness constitution as a significant risk factor for CVD. Future research should further explore the underlying mechanisms of the phlegm-dampness constitution in CVD and promote its application in clinical prevention.

背景

心血管疾病(CVD)是全球主要的死亡原因之一,随着代谢性疾病的增加,其发病率呈上升趋势。痰湿体质是CVD的潜在风险因素,但其与CVD之间的关系尚未得到充分的流行病学验证。

目的

本研究构建TANS01变量表征痰湿体质并探究与CVD的风险关系,评估痰湿体质在人群中的CVD风险。

方法

纳入美国国家健康与营养调查(NHANES)数据库2003—2016年的7个调查周期的15 279例参与者,构建二分类指标TANS01以表征痰湿体质,并通过流行病学分析探究新型指标TANS01与CVD发病率的潜在关系,采用单因素或多因素Logistic回归分析探究TANS01与各个心血管结局变量的关系。

结果

根据TANS01指标分类将研究对象分为痰湿体质人群6 437例,非痰湿体质人群8 842例。非痰湿体质人群与痰湿体质人群比较,年龄、年龄分组、性别、吸烟、饮酒、高血压、超重、血脂异常、饮食偏嗜、运动偏少、心力衰竭、BMI、白细胞计数(WBC)、红细胞计数(RBC)、血红蛋白(Hb)、糖化血红蛋白(HbA1c)、空腹血糖(FPG)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、尿酸(UA)、肌酐(Cr)比较,差异有统计学意义(P<0.05)。单因素Logistic回归分析结果显示,TANS01是心力衰竭(OR=2.67,95%CI=2.04~3.49,P<0.001)、冠心病(OR=2.04,95%CI=1.66~2.51,P<0.001)和心绞痛(OR=2.29,95%CI=1.72~3.03,P<0.001)的危险因素,多因素Logistic回归分析结果显示,在控制混杂因素(年龄、性别、吸烟和饮酒)后关联依然稳健。亚组分析结果显示,TANS01在不同性别、年龄、吸烟、饮酒、糖尿病、高血压患病情况中均为心力衰竭的危险因素(P<0.05),在不同亚组间预测效能差异无统计学意义(P交互>0.05);TANS01在不同性别、年龄、吸烟、饮酒情况中均为冠心病及心绞痛的危险因素(P<0.05),在糖尿病和高血压患者中TANS01对冠心病及心绞痛预测价值无统计学意义(P>0.05)。

结论

本研究结果表明TANS01与心力衰竭、冠心病和心绞痛等CVD的风险显著相关,为痰湿体质与CVD的关系提供了流行病学证据,支持痰湿体质作为CVD的重要风险因素。未来研究应进一步探索痰湿体质对CVD的作用机制,并推动其在临床预防中的应用。
