早发冠心病患者共病情况及共病模式研究

Abstract: Background Along with the continuous change of lifestyle,the incidence and hospitalization rate of coronary heart disease are increasing and becoming younger in average age,patients are also generally suffering from other chronic diseases,namely facing the problems of comorbidities but currently the relevant research in China is still relatively lacking. Objective To understand the prevalence and patterns of comorbidities in premature coronary heart disease,and to explore the correlation between comorbidities,in order to provide reference for early detection and management of comorbidities in premature coronary heart disease in China. Methods Electronic medical record data of 5 124 patients (males ≤ 55 years old,females ≤ 65 years old) admitted to the Department of Cardiology at Bethune Second Hospital of Jilin University from 2010 to 2022 were collected and analyzed to determine the current status of comorbidities in premature coronary heart disease. SPSS 26.0 statistical software and Python 3.11.0 were used,along with the Apriori algorithm,to mine strong association rules for premature coronary heart disease and explore the comorbidity patterns. Results Thirty five notable comorbidity patterns observed among the 13 chronic diseases. The binary comorbidity pattern of premature coronary heart disease takes "hypertension" as the core comorbidity,which can be accompanied by anemia,kidney disease,stroke,etc. The binary comorbidity pattern takes "anemia + kidney disease" as the core comorbidity,which may be accompanied by diabetes and lung disease,etc. The quaternary comorbidity pattern takes "diabetes + hypertension" as the core comorbidity,which may be accompanied by stomach disease,liver disease,etc. According to the results of the association rule and cluster analysis,hypertension,diabetes,dyslipidemia,anemia,and kidney disease all had significant comorbidity rates,with a complex comorbidity relationship. Conclusion The predominant comorbidity disease in the population of premature coronary heart disease is hypertension,and the comorbidity pattern of "anemia + kidney disease" of early-onset coronary heart disease can be examined in future studies. The management of early-onset coronary heart disease can increase the management effect by improving screening and comorbidity prevention,as well as refining the research population.

摘要: 背景 随着生活方式的不断改变,冠心病发病率与住院率不断上升且越来越向年轻化发展,人们也普遍罹患其他慢性病即存在共病问题,但目前国内相关研究相对缺乏。目的 了解早发冠心病共病患病情况及共病模式,并探索共病间的关联性,为我国共病筛查和早发冠心病共病管理提供参考。方法 收集2010—2022年吉林大学白求恩第二医院心内科收治的5 124例(男性≤55岁,女性≤65岁)电子病历数据,分析早发冠心病共病患病现状。通过使用SPSS 26.0统计软件与Python 3.11.0利用Apriori算法挖掘早发冠心病强关联规则,探索早发冠心病共病模式。结果 早发冠心病共病患病率最高的前3位分别为高血压(3 707例,72.35%)、血脂异常(2 134例,41.65%)和糖尿病(1 811例,35.34%)。13种慢性病共形成了35个有意义的共病模式。早发冠心病二元共病模式以“高血压”为核心共病,可能伴有贫血、肾脏疾病、卒中等。三元共病模式以“贫血+肾脏疾病”为核心共病,可能伴有糖尿病、肺部疾病等。四元共病模式以“糖尿病+高血压”为核心共病,可能伴有胃部疾病、肝脏疾病等。结合关联规则与聚类分析结果显示高血压、糖尿病、血脂异常、贫血、肾脏疾病共患率较高,存在复杂的共患关系。结论 早发冠心病人群中共病以高血压为主,在未来研究中可针对早发冠心病的“贫血+肾脏疾病”共病模式进行探索。
