Abstract: With the gradual maturing of technologies such as cloud computing,high-speed mobile Internet and Internet of Things,to realize the "Ubiquitous Healthcare" in the health field is not only an irresistible trend,but also a realistic possibility.According to the community health management theory and service-oriented system structure,the study proposed the designing scheme and implementation method of a management service platform for health information collection and big data analysis of mobile community of chronic disease based on cloud computing.With this platform,community residents can enjoy the convenient and quick self-management service of health,and community healthcare staff can provide timely and efficient service to the chronic disease patient or those potentially high-risk individuals precisely.The paper hopes to provide theoretical references for researches of ubiquitous healthcare and “be ubiquitous” of chronic disease prevention and control.

摘要： 随着云计算、高速移动互联网及物联网等技术的不断成熟，医疗卫生领域实现“泛在卫生保健”（Ubiquitous Healthcare）既是大势所趋，也具备现实可能。本研究依据社区健康管理理论和面向服务的体系架构，提出了一个基于云计算的慢性病移动社区健康信息采集和大数据分析管理服务平台的设计方案和实现方法。通过该平台，社区居民可享有方便快捷的自我健康管理服务，社区卫生服务者可以精准地向慢性病患者或潜在高危个体提供及时高效的服务，以期为泛在卫生保健和慢性病防治的“u化”研究提供理论依据。
