青年甲状腺功能亢进症并发反复脑梗死一例报道及文献复习

摘要: 目前,关于青年甲状腺功能亢进症患者并发脑梗死的研究报道较少见,相关发病机制亦未明确。本文报道了1例4年内3次出现脑梗死及进行性颅内动脉狭窄、闭塞的青年女性甲状腺功能亢进症患者的诊疗经过,并复习了相关文献,以总结其临床特征及发病机制等。鉴于积极控制甲状腺功能亢进症可有效改善脑梗死患者神经功能缺损症状并预防脑梗死复发,因此建议青年脑梗死患者积极筛查甲状腺功能亢进症,并在发现甲状腺功能亢进症时进一步行颅脑磁共振成像或脑血管影像学检查,尽量做到早发现、早治疗。

Abstract: Research reports about cerebral infarction in young adults with hyperthyroidism are few so far,and related pathogenesis is not clear.This paper reported the diagnosis and treatment of a young female patient with hyperthyroidism who had three times of cerebral infarction with progressive intracranial artery stenosis and occlusion within 4 years,and reviewed relevant literatures to summarize the clinical features and pathogenesis of this disease.Since active control of hyperthyroidism can effectively improve the accompanied symptoms of neurological deficits and reduce the risk of cerebral infarction recurrence,it is recommended that young patients with cerebral infarction should actively participate in hyperthyroidism screen,and if hyperthyroidism is found,further cranial magnetic resonance imaging or cerebrovascular imaging examination should be conducted,trying to achieve early detection and treatment as much as possible.
