上肢功能测试量表的汉化及其在脑卒中患者中的

信效度研究

【摘要】 背景 功能评估是康复治疗的基础，可协助临床制订康复方案，提高康复效率。目前用于脑卒中后
上肢功能评估的量表较少且维度单一。目的 汉化上肢功能测试量表（TEMPA），并评价该量表在脑卒中患者中的信
度和效度。方法 遵循 Brislin 原则，按照直译、回译、审查、文化调试、预试验的程序对英文版 TEMPA 进行翻译及
修订，形成中文版 TEMPA。于 2021 年 8 月至 2022 年 1 月招募在唐山市工人医院康复科就诊的 40 例脑卒中患者，评
价者分别用中文版 TEMPA、上肢运动功能 Fugl-Meyer 评定法（FMA-UE）、简易上肢功能评定量表（STEF）对患者
上肢功能进行评估，并于首次评估后 1 周内，使用 TEMPA 进行第 2 次评估。采用组内相关系数（ICC）检验 TEMPA
各维度的评估者内信度和评估者间信度；采用 Cronbach's α 系数检验 TEMPA 的内部一致性信度；对患者的中文版
TEMPA 各维度总分和各条目得分分别与 FMA-UE、STEF 得分进行 Pearson 相关分析，检验 TEMPA 的效标效度。结
果 TEMPA 功能评级总分的评估者内信度 ICC=0.992，TEMPA 功能评级总分的评估者间信度 ICC=0.982。TEMPA 任
务分析总分的评估者内信度 ICC=0.998，TEMPA 任务分析总分的评估者间信度 ICC=0.999。TEMPA 功能评级维度的
Cronbach's α 系数为 0.858。TEMPA 9 项任务在执行速度维度得分与 STEF 患侧得分呈负相关（P<0.05），相关系数
为 -0.785~-0.460。TEMPA 功能评级维度总分、任务分析维度总分与 FMA-UE、FMA-UE 腕手部分分数、STEF 患侧得
分均呈正相关（P<0.05），相关系数均在 0.7 以上。结论 中文版 TEMPA 具有良好的信度和效度，可用于测量脑卒中
患者上肢功能。

【Abstract】 Background Functional assessment is a foundation for rehabilitation treatment，which contributes
to the development of the rehabilitation program and the improvement of rehabilitation outcomes. However，there are only few
assessment scales with unvaried domains for upper extremity function after stroke. Objective To develop the Chinese version
of Upper Extremity Performance Test（TEMPA）and to explore its reliability and validity in stroke patients. Methods We
translated and revised the English version of TEMPA to a Chinese version according to the Brislin's translation procedures of
forward-translation，back-translation，review，cultural adaptation and a pre-test. Then from August 2021 to January 2022，
the Chinese version of TEMPA，Fugl-Meyer Assessment of Upper Extremity（FMA-UE） and Simple Test for Evaluating
Hand Function（STEF） were used to evaluate the upper limb function of 40 patients with stroke recruited from Department of
Rehabilitation，Tangshan Workers' Hospital . One week after the first evaluation，the Chinese version of TEMPA（TEMPA-C）
was used to evaluate their upper limb function again. The test intra-rater reliability and inter-rater reliability of each dimension
of the TEMPA-C were tested by intraclass correlation coefficient（ICC）. The internal consistency reliability of TEMPA-C was
