Recent Advances in Home Care Assessment Tools for Stroke Patients

Abstract

Abstract: Recent studies about home care，a link in continuous care，are increasing in stroke patients as an important type of intermediate care.Community-based home care for stroke patients is developing，focusing on home care assessment systems，patient discharge preparation services，and the development and application of family functions，and so on.However，there are relatively few studies on assessment tools for home care.We introduced the reasons for assessing home care for stroke patients，appropriate assessment time，and specific available assessment tools，aiming to provide the community nurse with a simple and effective tool with high accuracy and specificity for assessing home care for stroke patients to promote the effectiveness of home care，and to offer a theoretical basis for the development of appropriate home care assessment tools.

近年来，家庭作为脑卒中患者连续性照护的重要中间层级，相关研究逐渐增多，以社区为基础的脑卒中家庭护理不断发展，目前集中于居家评估系统、患者出院准备服务、家庭功能的开发和应用等研究。然而，关于家庭护理的评估工具研究相对较少。因此，本研究就脑卒中患者家庭护理评估背景、评估时机、特异性评估工具进行介绍，旨在为社区护士提供准确度高、特异性强、简单有效的脑卒中患者家庭护理评估工具，促进脑卒中患者家庭护理的有效性，同时为编制合适的家庭护理评估工具提供理论依据。
