Abstract: The patient-centered and family oriented community health service center is a new way to promote the advanced care planning（ACP），and it is an important part of community palliative care. There have been practical experience and theoretical discussion in some developed countries and regions. At present，the research on community ACP in China is still in its infancy. We need to learn from the advanced ideas and experience of foreign community ACP development to explore the ACP development model suitable for the Chinese community. Based on the perspective of community，this paper expounds the current situation of ACP development at home and abroad，analyzes the advantages of community ACP implementation combined with the current situation of community in China，and puts forward some suggestions for the development of ACP in Chinese communities，so as to provide reference for the research and practice of ACP in domestic communities.

摘要： 以患者为中心、家庭为导向的社区卫生服务中心是预立医疗照护计划（ACP）推广的新途径，是社区姑息护理的重要组成部分，在一些发达国家和地区已有实践经验和理论探讨。目前我国对社区ACP的相关研究仍处于起步阶段，需通过借鉴国外社区ACP发展的先进理念和经验来探索适合我国社区的ACP发展模式。本文基于社区的视角，阐述国内外ACP发展现状，结合我国社区现况分析社区ACP推行的优越性，提出ACP在我国社区的发展建议，以期为国内社区开展ACP研究与实践提供参考。
