Using Beers Criteria to Manage Medication Regimens for Elder Patients with Multimorbidity in Primary Care：a Case Study

Abstract

Abstract: It is increasingly common to treat elderly comorbid patients with polypharmacy in community-based general practice.How to formulate effective，safe and customized treatment plan for elderly comorbid patients is crucial for general practitioners in clinical practice.In this paper，the community general practitioner takes the polypharmacy for an elderly cerebral infarction patient with multiple diseases who is transferred from hospital to home-based care.This paper puts forward suggestions of treatment adjustment for the elderly patient by applying the medication use based on Beers criteria，so as to reduce the adverse drug reactions，and provide reference for community general practitioners to formulate multi-drug treatment programs for senile comorbidities.

摘要： 老年共病患者多重用药在社区全科诊疗中越来越常见，如何结合患者及家庭情况，为老年共病患者制定有效、安全、个性化的治疗方案是全科医生临床工作中的重要任务。本文详细介绍了1例医院转诊至社区建立家庭病床的老年脑梗死合并多种疾病患者，社区全科医生根据Beers标准的药物管理原则，分析患者的多重用药情况，调整该患者的治疗方案，减少了药物的不良反应，以期为社区全科医生制定老年共病多重用药治疗方案提供参考。
