Analysis and Countermeasure of Human Resources of Community Health Service Centers in the Small and Medium-sized Cities in Midwest China

中西部地区中小城市社区卫生服务中心人力资源现状分析及对策

Abstract: Objective To understand the current situation of human resources of community health service institutions in the small and medium-sized cities in Midwest China,and to analyze existing problems and put forward corresponding suggestions.Methods Luohe City of Henan,Ningguo City of Anhui,Hancheng City of Shaanxi and Panzhihua City of Sichuan were selected by purpose sampling from June to July 2017.Human resources allocation of community health service institutions were investigated by self-made questionnaire.Results In the above four cities,the number of health technical personnel per 10 thousands(5.03),professional (assistant) physicians per 10 thousands (2.11),general physicians per 10 thousands (0.47) were less than those of national average (5.78,2.43,1.51,respectively).From 2014 to 2016,annual average growth rate of number of human resource with bachelor's degree or above and senior title were 1.11%,18.08% in four cities,which were lower than those of township hospitals (24.67%,44.78%).Conclusion The human resources of community health service centers in the small and medium-sized city in the Midwest China have problems such as total amount deficiency,slow growth,unreasonable structure and difficulty in introducing high-level,high-quality talents.The government should pay more attention to the development of community health service by enhancing job attractiveness of general practitioner and carrying forward the medical partnerships to delegate high quality resources,expanding recruitment right of community health service centers,establishing a policy dynamic adjustment mechanism suitable for the development of community health service.

摘要: 目的 了解中西部地区中小城市社区卫生服务中心人力资源现状,分析存在的问题并提出相应的建议。方法 2017年6—7月,采取立意抽样法,选取河南省漯河市、安徽省宁国市、陕西省韩城市、四川省攀枝花市为调查地区。采取自制问卷,收集调查地区社区卫生服务中心人力资源配置情况。结果 调查地区社区卫生服务中心的每万人口卫生技术人员数(5.03人)、执业(助理)医师数(2.11人)、全科医生数(0.47人),均低于全国平均水平(5.78人、2.43人、1.51人)。2014—2016年社区卫生服务中心本科及以上学历、高级职称人员年均增长率分别为1.11%、18.08%,低于同期乡镇卫生院年均增长率(24.67%、44.78%)。结论 中西部地区中小城市社区卫生服务中心人力资源存在总量不足、增长缓慢、结构不合理,高层次、高素质人才引进困难等问题。政府应重视社区卫生服务发展,增强全科医生岗位吸引力,纵深推进医联体,促使优质资源下沉,扩大社区卫生服务中心招聘自主权,建立适合社区卫生服务发展的政策动态调整机制。
