痔的注射治疗在中国的应用现状及共识--一项横断面研究

Current Status and Consensus on the use of InjectableTreatment for Haemorrhoids in China -A cross-sectional study

摘要: 背景 痔疮是肛肠科常见疾病之一,注射治疗是常用治疗方法,但目前关于注射治疗的应用深度、规范应用情况等方面的全国性数据较为缺乏,因此有必要开展此项调查研究。目的 探究当前痔注射治疗在中国开展的深度和广度、规范性应用情况、有效性和安全性和未能开展的原因。方法 从2022年10月到2023年7月对中国医药教育协会肛肠疾病专业委员会的委员发布线上问卷,自行设计问卷内容,共37个问题,其中包括人口学资料、治疗痔的方法,注射治疗应用情况、并发症等问题,应用SPSS进行数据统计分析。结果 共回收335份问卷,有效问卷312份(回收率93.1%),超过60%的医生已实施500例以上的注射治疗,且各级医院无显著性差异,对Ⅰ~Ⅱ度内痔更倾向于注射治疗,注射治疗复发率低,副作用少,但仍有一些并发症。结论 痔注射疗法在中国应用广泛,其复发率低,副作用小,适合在各级医院进行推广,不过部分医生对其适应症不了解,需加强相关方面培训,确保治疗规范性,此外大部分医生将痔注射和其他术式联合使用,仍需要继续推广痔注射的单独应用。

Abstract: Background Haemorrhoids are one of the common diseases in anus and intestinal department, and injection therapy is used as a recommended treatment, but there is a lack of national data on the depth of application and standardised application of injection therapy, which makes it necessary to carry out this research study. Objective To explore the depth and breadth of current haemorrhoidal injection treatment in China, its standardised application, effectiveness and safety, and the reasons for its failure to be carried out.Methods An online questionnaire was issued to the members of the Professional Committee of Anal and Intestinal Diseases of the Chinese Association of Pharmaceutical Education from October 2022 to July 2023, and the content of the questionnaire was designed by ourselves, with a total of 37 questions, which included demographic information, methods of treating haemorrhoids, the application of injectable treatments, and complications, etc., and the data were statistically analysed by applying SPSS.Results A total of 335 questionnaires were recovered, 312 valid questionnaires (recovery rate 93.1%), more than 60% of doctors have implemented more than 500 cases of injection treatment, and there is no significant difference between hospitals at all levels. Injection treatment is more preferred for Ⅰ ~ Ⅱ degree of internal haemorrhoids, with a low recurrence rate of injection treatment and fewer side-effects, but there are still some complications.Conclusion Haemorrhoidal injection therapy is widely used in China, with low recurrence rates, few side effects, making it suitable for promotion at all levels of hospitals, although some doctors are unaware of its indications and need to be trained in relevant aspects to ensure standardisation of treatment; in addition, most doctors use haemorrhoidal injections in conjunction with other surgical procedures, and there is a need to continue to promote the use of haemorrhoidal injections on their own.
