社会支持与社区人群抑郁症关系的横断面研究

摘要: 背景 抑郁症是全球范围内日益严重的公共卫生问题,目前根据《精神障碍诊断与统计手册(第五版)》使用人格障碍定式访谈(SCID)诊断的抑郁症与社会支持之间关系尚不明确。目的 探讨社会支持与社区人群抑郁症的关系与改进策略。方法 于2022年9月-2023年11月共收集参加健康体检的河北省唐山市开滦马家沟社区人群2549人,采用横断面研究的方法。建立Logistic回归模型分析社会支持与抑郁症之间的关系,使用Lasso回归分析获得变量重要性,使用亚组分析进一步分析社会支持与抑郁症的关系。结果 本研究共纳入符合研究纳入标准与排除标准2189人,其中患抑郁症312人(14.25%),未抑郁症1877人(85.75%)。在调整了潜在混杂因素后,均得出社会支持水平下降与抑郁症患病风险增加相关。与高水平社会支持相比,中等水平社会支持与低水平社会支持的OR(95% CI)分别为1.535(1.072~2.198)、4.993(2.823~8.830),并且这种关系在各亚组中均存在(P<0.05)。结论 在该社区人群中社会支持水平的提高与降低抑郁症的患病风险相关,应采取相关措施提高社区人群社会支持的水平。

Abstract: Background Depression is an increasingly serious public health problem worldwide, and the relationship between depression diagnosed using personality disorder formulaic interviews (SCID) and social support according to the Diagnostic and Statistical Manual of Mental Disorders (Fifth Edition) is currently unclear. Objective Explore the relationship and improvement strategies between social support and depression in community populations. Methods From September 2022 to November 2023, a total of 2549 people from Majiagou Community in Kailuan, Tangshan City, Hebei Province were collected for health examinations, Logistic regression model to analyze the correlation between social support and depression, use Lasso regression analysis to obtain variable importance, and further analyze the relationship between social support and depression using subgroup analysis. Results This study included a total of 2189 individuals who met the inclusion and exclusion criteria, including 312 individuals (14.25%) with depression and 1877 individuals (85.75%) without depression. After adjusting for potential confounding factors, it was found that a decrease in social support levels is associated with an increased risk of depression. Compared with high-level social support, the OR (95% CI) of moderate level social support and low-level social support are 1.535 (1.072~2.198) and 4.993 (2.823~8.830), respectively, and this relationship exists in all subgroups. Conclusion The improvement of social support level among the community population is related to the reduction of the risk of depression, and relevant measures should be taken to enhance the level of social support among the community population.
