Abstract:

Background

Hypertension, diabetes mellitus, and dyslipidemia are prevalent chronic metabolic disorders and major risk factors for cardiovascular diseases. These conditions frequently coexist and demonstrate high population prevalence. A systematic evaluation of their epidemiological characteristics and associated factors is crucial for early disease prevention and control.

Objective

To systematically assess the prevalence and associated factors of comorbid hypertension, diabetes, and dyslipidemia among Chinese adults.

Methods

A systematic literature search was performed in databases including China National Knowledge Infrastructure (CNKI), Wanfang Data, VIP, CBM, PubMed, Embase, Web of Science and Cochrane Library, to retrieve relevant studies on the prevalence of the co-occurrence of hypertension, diabetes mellitus and dyslipidemia among Chinese adults. The search was restricted to publications from database establishment to August 2025. Meta-analysis was performed using Stata 18.0 software.

Results

A total of 18 cross-sectional studies involving 625 718 patients were ultimately included. The meta-analysis showed that the prevalence of comorbidity of hypertension, diabetes, and dyslipidemia among Chinese adults was 6.2% (95%CI=4.9%-7.4%). Specifically, overweight (OR=2.02, 95%CI=1.74-2.35), obesity (OR=3.71, 95%CI=2.85-4.81), central obesity (OR=2.47, 95%CI=1.98-3.07), male (OR=1.62, 95%CI=1.35-1.96), alcohol drinking (OR=1.45, 95%CI=1.25-1.68), family history of diabetes (OR=3.88, 95%CI=1.57-9.58), smoking (OR=1.54, 95%CI=1.26-1.88), marital status classified as non-married (divorced, separated, or widowed) (OR=1.88, 95%CI=1.55-2.27) were all influencing factors for comorbidity of hypertension, diabetes, and dyslipidemia (P<0.05).

Conclusion

The comorbidity prevalence of hypertension, diabetes, and dyslipidemia is relatively high among Chinese adults, and is influenced by factors such as overweight, obesity, central obesity, male, alcohol drinking, family history of diabetes, smoking, and marital status classified as non-married (divorced, separated, or widowed). Early identification and effective management of related controllable factors can better assist clinical healthcare providers in early recognition, potentially reducing their risk.

其中,超重(OR=2.02,95%CI=1.74~2.35)、肥胖(OR=3.71,95%CI=2.85~4.81)、中心性肥胖(OR=2.47,95%CI=1.98~3.07)、男性(OR=1.62,95%CI=1.35~1.96)、饮酒(OR=1.45,95%CI=1.25~1.68)、糖尿病家族史(OR=3.88,95%CI=1.57~9.58)、吸烟(OR=1.54,95%CI=1.26~1.88)、婚姻状况非在婚(离异、分居、丧偶)(OR=1.88,95%CI=1.55~2.27)均是高血压、糖尿病、血脂异常三病共患的相关因素(P<0.05)。
