Abstract: Hypertension,type 2 diabetes and dyslipidemia often occur together and are collectively referred to as the“three high”. As the main battlefield of chronic disease prevention and control,primary healthcare institutions receive a large number of“three high” patients every day. This article presents the specific steps of“three high” management through the case of a“three high” patient to provide ideas for the management of such patients. After intervention,the patient's blood pressure,blood glucose,blood lipids and weight have all met the treatment targets,and the risk factors,complications and comorbidities have all been well controlled. The patient's adherence to medication has improved,and their ability to manage themselves has been enhanced,which helps to enhance the management effect of“three high” patients and further standardize the community management model of“three high” patients.

摘要: 高血压、2型糖尿病和血脂异常常同时发病,被统称为“三高”。基层医疗卫生机构作为慢性病防治的主战场,每天会接诊大量“三高”患者。本文通过一例“三高”患者的诊疗过程展示三高管理的具体步骤,以期为“三高”患者管理提供思路。通过干预后,该患者血压、血糖、血脂、体质量均已达标,危险因素、并发症和合并症均控制良好,用药依从性改善,自我管理能力增强,“三高共管”有助于提升的“三高”患者的管理效果,进一步规范“三高”患者的社区管理模式。
