Recent Developments in Assessment Tools for Discharge Planning for Elderly Patients

Abstract

Abstract: The post-discharge readmission rate of elderly patients is high，therefore making a discharge plan based on comprehensive pre-discharged evaluation is the guarantee for these patients receiving high-quality continuous care after the discharge.Some researchers have reviewed discharge planning assessment tools used abroad，but there is still a lack of reviews targeting at elderly inpatients，a population with special characteristics.We reviewed discharge planning assessment tools for elderly patients used at home and abroad，mainly focusing on discharge risk assessment tools and comprehensive assessment tools，hoping to help healthcare workers make an appropriate choice of discharge planning assessment tools.

摘要： 老年患者出院后再入院率高，而通过细致评估后制定出院计划是患者出院后获得高质量延续性护理的重要保证。既往已有研究者对国外出院计划评估工具的使用进行了综述，但缺乏专门针对老年患者的出院计划评估工具的相关报道。由于老年患者具有特殊性，本文主要从老年患者出院风险筛查和综合性评估的角度对国内外使用的老年患者出院计划评估工具进行综述，从而为相关医务人员选择合适的出院计划评估工具提供依据。
