Abstract: Cardiac rehabilitation has been strongly recommended by international guidelines for its beneficial effects.Referral model between tertiary hospitals and community health service centers is worth exploring，because the community-based cardiac rehabilitation will be the major model.At present，the main hospitals in China to carry out cardiac rehabilitation are tertiary hospitals.In the case of limited rehabilitation hospitals，it is necessary to study the referral mode of cardiac rehabilitation between third-level hospitals and community health centers.This review mainly focused on the existing problems and the solutions of referral model based on the Internet plus tertiary hospitals and community health service centers.We，also put forward personal constructive suggestions. that medical treatment partnership and alliance based on the Internet are feasible referral model of cardiac rehabilitation.The aim was to promote the development of cardiac rehabilitation referral model between third-level hospitals and community health centers based on the Internet.

摘要： 由于心脏康复对心血管病的诸多益处，其已被诸多指南推荐用于心血管病患者的治疗，未来社区卫生服务中心将是心脏康复的主要场所，而目前国内主要是三级医院开展了心脏康复，在专科康复医院数量有限的情况下，需探索三级医院与社区卫生服务中心心脏康复转诊模式。本文就目前互联网+三级医院与社区卫生服务中心心脏康复转诊模式存在的问题及推进方法进行了阐述，主要提出了基于互联网+的医联体及联盟模式是可行的心脏康复转诊模式，旨在为推进互联网+三级医院与社区卫生服务中心心脏康复转诊模式的发展提供帮助。
