基于“虚损-瘀毒”与线粒体自噬的关系探讨阿尔茨海默病的中医病因病机

Abstract: The pathogenesis of Alzheimer's disease is complex,and the role of mitochondrial damage in AD has become a current research hotspot. In pathological conditions,mitophagy dysfunction leads to pathological accumulation of damaged mitochondria in the body,resulting in insufficient ATP production,continuous release of mitochondrial free radicals and harmful substances,inducing neuronal cell apoptosis and exacerbating AD damage. The pathogenesis of "deficiency-stasis toxin" runs through the occurrence and development of AD. AD begins with deficiency and damage of the five organs,leading to phlegm turbidity and blood stasis. Over time,it produces toxic pathogens,clears the orifices,and causes the loss of vital energy. From the perspective of traditional Chinese medicine,there is a certain relationship between the pathological mechanism of mitochondrial autophagy and the etiology and pathogenesis of " deficiency-stasis-toxin " in traditional Chinese medicine. Based on the relationship between the two,under the guidance of the pathogenesis of deficiency-stasis-toxin " in traditional Chinese medicine,further explore traditional Chinese medicine to improve mitochondrial autophagy,and provide new ideas for the treatment of Alzheimer's disease with traditional Chinese medicine.

摘要: 阿尔茨海默病(AD)的发病机制较为复杂,目前线粒体损伤在AD中的作用成为当前的研究热点。在病理状态下,线粒体自噬功能紊乱导致受损线粒体积聚,ATP生成不足、自由基及有害物质释放,诱导神经元细胞凋亡,加剧AD损伤。“虚损-瘀毒”病机贯穿AD发生、发展的始终。AD始于五脏虚损,进而出现痰浊、血瘀,日久酿生毒邪,上蒙清窍,导致神机失用。从中医视角看,线粒体自噬病理机制与“虚损-瘀毒”的病因病机密切相关。基于两者的关系,结合“虚损-瘀毒”病机理论,探索改善线粒体自噬的中药,为中医药治疗AD提供新思路。
